Email and web forms are ordinary communication channels rather than secure clinical systems. That is worth knowing before you decide how much to include in a first message.
A general description — “I have had trouble sleeping for about a year and would like to know whether this is worth exploring” — is entirely sufficient to begin. Detailed medical history, diagnoses, medication lists and anything else you would consider sensitive are better discussed directly rather than written into a form.
